Why rescuers die
HSE statistics show that around two-thirds of confined space fatalities are would-be rescuers — colleagues going in to help without equipment, training or atmosphere checks. The first minute is the dangerous one.
A real rescue plan removes the temptation to improvise.
Match the rescue method to the space
Vertical entry with self-rescue retrieval line: works for many short-duration entries.
Non-entry rescue with tripod and winch: the default where geometry allows.
Entry rescue with breathing apparatus: only with a trained rescue team on standby and a documented retrieval plan.
Pick the method during the risk assessment, not in the moment.
Drill cadence
Quarterly drills with the actual team, the actual equipment, and a realistic casualty weight. Annual is theatre.
Capture drill performance: time to retrieval, communication failures, equipment defects. Feed every finding into the corrective action register.
Evidencing competence
Inspectors don't want certificates — they want to see that the named entrants and the named rescuers practised together this quarter. Keep training records, drill records, and equipment inspection records linked to the same permit template.
